Appointments can be made for you to be seen at either Subiaco or Carine.
At Your Appointment
Please bring your referral from your GP or Specialist, Medicare card, private health fund details and any relevant x-rays, CT scans or MRI’s that could be useful during your consultation. If this is your first consultation with Mr Sowman please arrive 10minutes before your scheduled appointment to complete the appropriate paperwork. Consultation fees are payable on the day of your appointment and can be made by cash, EFTPOS, Visa or MasterCard.
