Information that is held within your medical file is collected in order to evaluate and assess the cause of your condition, provide a diagnosis and appropriate treatment plan. Every staff member at Spine West will have access to this information.
Information may be provided to other administrative professionals, such as treating doctors, pathology services, radiology services and other specialists as deemed necessary by Dr Braad Sowman. Access to your personal information will be provided in accordance with the WA Legislation and Commonwealth Privacy Act 19.12.2001.
Information may be required for administrative purposes, such as medical indemnity insurance, provider information, billing, hospital bookings, Health Insurance funds, Health Insurance Commission (Medicare) and other financial organisations responsible for your care such as the Insurance Commission of Western Australia / Work Cover.
